3. Please indicate the number of employees in your organisation.  
4. How many of these employees identify as women?  
5. How many of these women identify as Black, Brown, Indigenous or women of colour (BIWoC)? There will be the possibility to choose “We do not have that data.”
6. Please indicate the number of people identifying as women that occupy leadership positions in senior management.  
7. Please indicate the number of people identifying as BIWoC that occupy leadership positions in senior management, should you have this data.
8. Please indicate the number of people identifying as women that occupy positions in governance or supervisory bodies. 
9. Please indicate the number of BIWoC identifying as women that occupy positions in governance or supervisory bodies, should you have this data.  
10. Our organisation aims to advance gender equality and increase diverse representation with internal policies that guide us in this process (yes/no).
